Pre-admission Form - Online submission

Registration

Please proceed to the Day Surgery Centre at Level 4 for registration. You may take Lift 1 and 2 next to our Chapel for direct access to the Day Surgery Centre.

Original Documents required

  • Identity Card or Birth Certificate (if under 12 years old) or Passport (for foreigners), Spouse’s/Parent’s Identity Card
  • Doctor’s Admission Letter
  • Previous medical health reports e.g. X-Ray, Laboratory Reports, Blood Test Results, etc.
  • Letter of Guarantee from your employer/insurance company if part or all of your medical expenses are to be borne by your company/insurance company
  • A list of all medications you are presently taking, and the dosage

Valuables

Please do not bring along excessive cash and valuables. Please leave your valuables at home as the hospital will not be able to accept responsibility for any losses.

Pre-surgery Preparation

Instructions for what you should and should not do will be provided by your surgeon prior to your surgery. Please ensure that you do not eat or drink as instructed by your doctor.

Waiting Areas

The hospital has a café and several designated waiting areas for visitors. Family and friends are invited to use these waiting areas and allow patients to rest adequately before discharge.

Payment

You can use Medisave to cover your day surgery expenses. Medisave withdrawal limit per day for day surgery is $300. In addition, surgery fees of between $150 and $5000 are claimable depending on the type of surgery. Any outstanding balance can be paid by cash, NETS or credit card on the same day of your operation.

Discharge

Please arrange for someone to accompany you home. You are not allowed to drive as the after effect of anaesthesia may not have completely worn off.
